We would like to reassure you that any personal information we collect as a result of your visiting this website is protected. We are aware that you need reassurance when visiting a website.
The viewing of this website does not constitute a dentist/ patient relationship. A relationship with the service provider will be governed by an agreement acceptable to both the client and the dentist.
In the event of an enquiry there is no binding obligation for the provider to answer. There is no obligation for a client sending a query as a result of a client dentist relationship.
We undertake to use your information for the purposes for which it was collected.
Information collected from you will be deemed necessary for us to provide optimum care. Any information that we obtain from you or others that relates to the health care that you have received and payment for health care is maintained under our Privacy Policy. By law we must ensure your privacy and adhere to the protected health information guidelines.
There are instances when your health care information will be disclosed or used for treatment, payment and evaluation purposes.
Consultation may be needed between practitioners in order to ensure the best course of treatment. Referrals may necessitate disclosure of protected health information. Information about your medical condition may be needed by your Health Care Provider or a third party to ascertain eligibility to provide treatment.
Your information may be used to conduct ongoing review and assessment within our practice, in order for us to ensure we are providing optimal patient care.
We undertake to protect any data collected from you for the purpose agreed on between the client and the dentist. Your data may be collated and backed up on our computer. It will be retained for as long as deemed necessary or by law, beyond the expiry of it’s purpose and will be destroyed in a manner whereby it will not be possible to retrieve it in part or whole.
We will use the information provided by you to contact you regarding appointments and to follow up on your progress. We may contact you by phone, e-mail, mail or SMS to remind you of an appointment or to suggest a course of treatment. We may leave such a message on an answer phone, your mobile or with a family member.
Using our discretion we may discuss your information to a family member or friend, identified by you, if they are responsible for your care or for payment. Only relevant information will be disclosed and you may object to such disclosures if you wish. If you are not available we will act in your best interests.
Similarly we will deny requests of access to your protected health information from other parties, if we deem in our professional judgements that it is unnecessary, would put you at risk or cause you harm.
There are circumstances when we may be required by law to share information. We are bound to release treatment information, for example with Veteran Affairs. Medical information can be disclosed to Public Health Bodies, Health Funds, Federal or State Agencies, Lawful Agencies, re subpoenas, Coroners and Funeral Directors.
You have the right to request restrictions on our disclosures however we are not required to acquiesce to such requests in all instances.
Any queries or concerns should be addressed in writing to the provider and we will endeavour to address these. We undertake to protect your privacy and not to materially change the stated terms in this Privacy Statement in any manner, which would render the meaning changed, or your protection compromised.
